Measure the two shoulders of a career professional and they will not match. Not merely in strength. In range, and in some cases in the shape of the bone itself.

The serve demands enormous outward rotation of the upper arm in the cocked position, then a violent reversal inward. Do that from childhood, while the bone is still growing, and the upper arm bone twists slightly along its length in response, which effectively gives the joint more outward rotation without moving anything else. It is a genuine adaptation and it is useful, because more range in the cocking phase means a longer runway to accelerate the racket.

The bill arrives at the other end of the arc. Total range across the joint tends to stay roughly fixed, so what is gained in outward rotation is usually lost in inward rotation, and the tissue at the back of the shoulder tightens progressively over years of decelerating the arm after contact. A shoulder with plenty of outward rotation and little inward rotation is the classic overhead athlete pattern, and it shifts the resting position of the ball of the joint slightly backwards in the socket.

Once that shift exists, the follow-through becomes the risky moment rather than the acceleration. Bringing the arm across the body after contact drives the joint into the very range that has been lost, and the soft tissue at the back takes the strain. Players report a pain that arrives after the ball has gone, which puzzles them because nothing felt wrong during the effort.

This is why the aging serve loses shape in a particular way. Not weaker first. Lower, and shorter through the finish, because the follow-through hurts and the body quietly shortens it. Shorten the follow-through and the deceleration is concentrated over less distance, which increases the load again. It is a loop, and it tightens.

The mildly annoying conclusion for anyone who wants a simple answer is that the very adaptation that makes a great server is also the one that will end him. It is not overuse in the crude sense of doing too much. It is a structural trade that begins at fourteen, pays off for fifteen years, and then presents its invoice in the range of motion nobody was measuring at the start.
