The injury is functional rather than structural
Concussion involves a disturbance of brain function following a force transmitted to the head. Standard imaging typically appears normal, since no visible structural damage is present.
Diagnosis therefore rests on clinical assessment of symptoms, cognitive performance and balance. There is no single objective marker that confirms or excludes it.
Because the evidence is functional, it must be gathered by observation and testing over time rather than captured in a single measurement.
Symptoms evolve after the event
Some effects appear immediately while others emerge over the following hours. An athlete assessed as normal shortly after an impact may develop symptoms later that evening.
That delay is the reason immediate clearance is avoided even when initial screening appears reassuring. The absence of symptoms at one moment is not conclusive.
Repeat assessment at defined intervals captures the developing picture. Comparison across those assessments carries more information than any one of them.
Baseline testing improves interpretation
Cognitive and balance performance vary considerably between individuals, so a result within a normal population range may still represent a decline for a particular athlete.
Testing athletes when healthy provides a personal reference point. Post-injury results are then compared against that individual baseline rather than against a general standard.
Baselines have limitations, since performance can vary with sleep, mood and effort. They inform the assessment rather than determining it.
Graduated return is part of the assessment
After an initial period of rest, activity is reintroduced in stages from light aerobic work through to full contact. Each stage is separated by a defined interval.
The progression tests tolerance as well as delivering conditioning. A return of symptoms at any stage indicates the brain has not recovered and the athlete steps back.
Because the stages take time, the process cannot be compressed into a single day regardless of how well the athlete feels. The interval is itself part of the test.
Why removal from play is immediate
An athlete showing signs of concussion is removed without waiting for confirmation, because a second impact before recovery carries substantially increased risk.
Sports have adopted independent assessors and video review for this reason, removing the decision from the athlete and the team's competitive interest.
Concussion management requires qualified medical supervision, and protocols vary between sports and jurisdictions. Anyone with a suspected head injury needs professional assessment rather than a general rule.
